Unveiling the Silent Assassin: Global Shadow of Hepatitis C and Its Devastating Toll 

Hepatitis C is the only major chronic viral infection for which a functional cure exists yet hundreds of millions remain undiagnosed. The gap between what medicine can do and what diagnostic systems deliver is where lives are lost.

A Curable Disease the World Is Still Losing To

Hepatitis C virus (HCV) is a single-stranded RNA virus, classified into six major genotypes (1–6) with distinct geographic distributions and varying therapeutic responses. Its clinical trajectory is defined by silence: the majority of infections produce no symptoms during the acute phase, allowing the virus to establish chronic infection before most patients are aware of exposure.

Among chronically infected individuals, up to 20% develop cirrhosis and once cirrhosis is established, hepatocellular carcinoma risk accumulates at 1–4% per year. HCV is now one of the leading drivers of liver transplantation and liver cancer globally. The clinical stakes are compounded by the fact that HCV co-infection with HIV or HBV dramatically accelerates liver disease, making accurate and early identification of every co-existing pathogen a clinical imperative, not an afterthought.

Cutting the Chain: Prevention Rooted in Systems and Awareness

HCV spreads almost exclusively through blood-to-blood contact. Intravenous drug use, unsafe injection practices in healthcare settings, unscreened blood products and inadequately sterilized medical instruments are the primary transmission routes. Consistent use of sterile, single-use needles and rigorous sharps disposal protocols in both clinical and community environments remain the most effective structural interventions available.

Mandatory nucleic acid testing of all donated blood and organ products has transformed blood safety in well-resourced settings and must be extended globally. Harm reduction programs for people who inject drugs when implemented with scale and without stigma have demonstrated measurable reductions in new HCV incidence. Routine screening of high-risk groups, including people with a history of injection drug use, those with unexplained liver enzyme elevation and HIV-positive individuals, is the highest-yield diagnostic intervention a health system can deploy.

Integrating HCV screening into primary care and HIV testing programs normalizes testing as a routine health behavior, dramatically shortening the time between infection and diagnosis.

RNA Over Antibodies: The Case for Molecular Testing

Anti-HCV antibody assays confirm prior exposure but cannot distinguish active from resolved infection, miss acute cases during the 8–12 week serological window and offer no quantitative information on viral replication. Quantitative real-time RT-PCR resolves all three limitations: HCV RNA becomes detectable within 1–2 weeks of infection, enabling diagnosis during the window period and supporting earlier clinical intervention.

For chronic HCV, RNA viral load is the foundation of treatment management. Baseline quantification determines treatment eligibility and duration; on-treatment RNA kinetics define virologic response; and Sustained Virologic Response, undetectable HCV RNA 12 weeks post-treatment is the internationally accepted definition of cure, achievable only through sensitive molecular confirmation. Syndromic multiplex panels that simultaneously test for HCV, HBV, HAV and HIV from a single sample further sharpen clinical decision-making in acute hepatitis presentations, preventing missed co-infections that alter therapy entirely.

SVR cannot be confirmed by serology alone. A sensitive real-time RT-PCR assay is the only clinically valid method to verify viral eradication after treatment completion.

Quantifying the Cure: TRUPCR® HCV Viral Load Kit

The TRUPCR® HCV Viral Load Kit by 3B BlackBio Dx Ltd. is a one-step real-time RT-PCR IVD for detection and quantification of HCV RNA across all genotypes and sub-genotypes in human serum or plasma (EDTA). The kit detects all six clinically relevant genotypes (1–6). An endogenous internal control runs in the tube, providing per-reaction quality monitoring without additional wells.

The diagnostic kit demonstrates excellent analytical and clinical performance for hepatitis C virus (HCV) detection and quantification. It achieves a sensitivity (limit of detection) of 20 IU/mL. The assay is calibrated to the 6th WHO International Standard (NIBSC 18/184). In a blinded clinical correlation study involving 130 patient samples compared against a CE-IVD certified reference method and sensitivity and specificity of 98.75% and 98% were reported respectively.
